RELEASE FORMS
Here are all of our release forms that we use for our student ministry. Our permission release form is used for all trips in the local Kansas City area, including some of the events we hold at our church. The medical release form is for all trips outside of Kansas City, although it is encouraged to turn one in anyway to increase the protection of your son or daughter. We encourage you to notarize this form for all trips outside of the state as it removes any uncertainty of issues during intake at any hospital and gives your child quicker access to medical help. There may also be additional forms for specific conferences or trips done in conjunction with another ministry or organization (such as C.I.Y. trips). Those trip releases will be available in this location as they become available (including a link to C.I.Y.'s electronic release form).
